Thomas James Smith III, 49, died after a crash on US-52 Northbound near Patterson Avenue in Winston-Salem on June 4, 2026.
Authorities said Smith drove a 2021 Dodge northbound when the vehicle left the roadway for reasons still under review.
The Dodge struck a safety barrier, returned to the roadway, and then collided with a 2012 Honda.
Emergency crews found Smith unconscious, provided aid at the scene, and transported Smith to a nearby hospital.
Medical teams worked to save Smith, but Smith later died from the injuries sustained in the crash.
The 59-year-old Honda driver did not sustain injuries, according to the available report.
